Immunization Exemptions
The California State University system allows for exemptions to immunization requirements based on a medical condition that is a contraindication to vaccination for the following vaccines: MMR (measles, mumps, and rubella), varicella (chickenpox), Tdap (tetanus, diphtheria, and pertussis), meningococcal conjugate (Serogroups A, C, Y, & W-135), meningococcal serogroup B and hepatitis B, and/or TB screening requirements.
What if I have an allergy or medical contraindication to a vaccine?
The CSU allows exemptions to immunization requirements based on a medical condition that is a contraindication to a the vaccine. Should you have an allergy or a contraindication, please download the Medical Exemption Request, ask your medical provider to complete and sign it, and then submit it via the “Health and Counseling Portal” within your MyCalPoly portal.
