COVID-19 Vaccine Resources & Information

There are many options available to students to receive their COVID-19 vaccine. COVID-19 vaccine appointments are free and don't require information about the recipient's immigration status. Please read below and find the option that works best for you.

Cal Poly Health Center

 

Make an appointment at the Cal Poly Health Center.

Students can call 805-756-1211 to make vaccination appointments.**

On-Campus Vaccination Clinics

 

**Campus Health and Wellbeing offers a limited number of COVID-19 vaccine appointments to Cal Poly students at the Health Center (Bldg. 27) on campus. The center will offer either the Pfizer or Moderna vaccine for any enrolled student who needs their first dose, second dose, or booster. Please note that students who receive vaccinations at CHW will need to upload their vaccination record on the COVID-19 Info tab of the Cal Poly Portal or the COVID-19 Vaccination Status Declaration

  • Appointments: Students can call 805-756-1211 to make vaccination appointments. 

  • What to bring: Bring your Poly Card or a photo ID and a completed vaccination consent and questionnaire. Completed vaccination consent form should be brought in to appointments, printed and in-hand. Electronic copies of this form will not be accepted. If this is a second-dose appointment, bring your vaccination card.

  • Parking: There are free, one-hour parking zones on the top level of the Grand Avenue parking structure for those with COVID-19 vaccination and testing appointments.

  • Cancellations: Call the Health Center at 805-756-1211 if you need to cancel your appointment.
