Medical Excuse Policy
Students being evaluated at Campus Health & Wellbeing for medical concerns are provided visit verifications through their Health and Counseling portal. Students, please follow these instructions to access your appointments:
- On your computer, go to your Portal.
- Click on Health and Counseling Portal.
- Under Appt. Scheduling, click “View Appts.”
- Once there, select the “Previous” tab.
- Take a screenshot. Crop or block out your “Reasons” for appointment so that the picture does not show sensitive information.
- Please include your name in the screenshot and ensure you've captured active status appointment. See image for details.
- This will serve as your class note. Please show this to your professors and inform them this is the only visit verification Campus Health & Wellbeing will provide.
- For employers: please schedule a visit with one of our providers and ask them to provide a work note.
- For prolonged illnesses: please obtain a note from the provider treating you for your illness at your earliest convenience. Students should also visit the Cal Poly Dean of Students' CARE team page for additional information and support.
